Big Help for big project needed !!!!!

Hello everyone this time i wanna make a big project so i need help
I want to make a Formular with quetions
so people can thoose how many textboxes apper how many tick boxes labels and
so what i need is
1 how to make that people can choose how many text boxes how many labels how many tickboxes they want
2 how to make it that when the person select everything theres a button where writes finish so when the person klicks finish
it creates an exe file with the questions and everything else
3 how to make that people can rename those labels
4 how to make it that when people fill the created formular the input get saved on a text file on c:\
so thanks for every help im using vb6
