Error When Using Contact Form

I am using this simple contact form which e-mails the user after completed. It has two files: form.html and send_form_email_.php
I downloaded these two files to my server and am receiving errors when trying to submit.

Can anyone advise?

What errors are you receiving, to start?

Looks to me like it's one of two reasons:

1. You didn't upload the php file to your server, double check it.
2. In your form tag, you aren't referencing the filename correctly (in your original post you called it send_form_email_.php, then in the form tag you referenced it as send_form_email.php and then in your post, you called it send_form_mail.php - that's 3 different names!), check the name of the actual file and get back to us.
