Uploading multiple images in database with same value from one field in form

Dear all,
I have a problem that I need to be helped with using one form and storing the images in database. I would like to upload multiple images with same title from "title" field.
Can somebody help me with this?

add a random number to the title so they become unique

Random number? no, just have a counter id field as primary key and then upload each image to a post with the title on each.
