Basic Help...first time coding...Compile multiple fields

I have created a crude html doc with 11 input fields and a submit button (which I put together just as an example of an idea I had)...what would be the best, or should I say easiest, way to be able to click the submit button and have it pull all the info from the fields and arrange it in an order that I specify? This way I can present my idea a bit better.

Again this is very crude as I have never done anything like this before. I will include the page in an attachment. Any advice is greatly appreciated.

Are you wanting to sort this with JavaScript on the client side, or on the server with ASP/.NET/PHP/other?
