I personaly like the head first series. For PHP and HTML thou I sugest just using W3Schools Online Web Tutorials

Agree completely with the W3Schools recommendation.

In terms of books, I found Visual Quickstart Guide: HTML, XHTML, and CSS very clear and well written.

I also agree with w3schools. I've learnt SO much there over the years. They are really, really good when you are starting out.

For PHP, you better download the PHP manual chm from PHP.net

Only needed to be downloaded if you will work without an internet connection, otherwise, seaching php.net is almost just as quick... Also, the chm wont include user contributions, I assume, which can be very helpful at times on how to use some functions.
