Basicly it will be like a survey, the page will have a question on it, the form fields will be: Answer (required), Name (required), Email (optional if phone is filled), Phone number(optional if email is filled),Zip code, Refferal Number (required), and also a text field for about a paragraph of text, to be sent to a predetermend email (which must not be visible in the code). Needs to check and make sure either the phone number is given OR the email. one or the other must be filled in.
My site is hosted on godaddy, and currently installed with php 5.
Extras that might warrant a bonus.
-Would also be nice if it has an auto responder to those that include an email address.
-Checks to validate phone numbers, email, zip.
-A very well written, safe, reliable script. that needs little to no modifications.

I will do this for $40 so long as your specifications do not exceed what you have posted above. However, I am unwilling to work with any theme or CSS - basically the submit forum will be raw HTML and you will need to incorporate it into your website theme.

Its not nessesary for it to follow any theme, plain html/php is fine, just needs to get the job done, do what is nessesary and cant be the "mailto()" thing, it must send from the site. not open a email client.
As long as it does what is above, I think I can add or manipulate simple things.

Yes, sure. It is easy.
My Stav Cinemas project sends mass emails to all our customers. Basically, it loops through each nonzero customer in the database and emails them a personalised message, with their own name and so on in the message body. "To Jordan, blah blah" etc.
