1) Operating System
2) OS Version (SP1, SP2, etc.)
3) Hardware - Memory, CPU Speed, etc.
4) Language (if other than english)
5) Bugs found
6) Overall Impression
7) What would you change?
8) What was missing?
9) What did, if anything, you like?
10) Would you pay for this? How much?

Maybe you should also add:
Resolution
Standard Font Size
Graphics Card Type
Just some suggestions that may help solve bug issues.

All good suggestions. I will add them to my list.
Instead of "What did you dislike most" I'll use "What would you like changed?"
