I cannot stress enough, that when optimizing a certain keyword, you should have it in your title tag. Unless you have a huge amount of links coming at you that has the keyword in the anchor text (searching "failure" on Google is a great example), it's important to have the keyword in the title. It just makes SEO easier.
1) H1 in my experiments make no difference unless the h1 font size relative to the remaining text in the page is big enough You do not need H1, if you set big font by yourself
2) You should have ALT tag for every image, it is going to be useful as crawlers can not see images.
3) Description tag is useful, it may make small difference and show up in google snippet sometimes. In my experiments, description tag makes difference but meta tag keywords does not.
4) title tag is the most important element in your page. In my opinion, all elements in the page on one side and title tag on side...
I thought any kind of meta tag didn't do anything because meta tags weren't used at all anymore.
