The program isn't called DDoS. DDoS is a type of attack, Distributed Denial of Service. Most of the times it's performed by bots/zombie boxes.

It can it just depends. The end goal of a Denial of Service attack is too deny people service to the site or the resource. This can be done in a variety of ways. One such way is the aforementioned ping method but many servers are no longer vulnerable to this. This is usually called a ping flood or icmp flood. (Icmp packets are ping packets) The attack can also be carried out by changing routing information so that the site can't be reached, or by changing dns information. Which is many times known as dns cache poisoning. A huge influx in otherwise legal requests to a site can also crash the server, which can be perpetrated as a dos attack. There are also ways to exploit an operating system and tcp/ip to carry out such an attack. You could also feasibly carry out a dos attack using arp poisoning, although this would have to occur on a LAN.

So a DDoS attack can be anything as long as the server is unreachable.

From what I understand, creating a lot of connections to a server (say web connections) will cause child processes of httpd to spawn to handle the load. If enough child processes are spawned, it will cause the servers memory to run out and spill over into the swap. This causes the server to run extremely slow and eventually crash.

That's very old, that type of attack ( pings ) doesn't work in this times, if you want to attack a website with floods and stuff you need a really good botnet. Why the pings don't work ? , because the request to the server is just a few kb , but if you use a botnet making a get request to the index or searching in the forum with a botnet in the search form this will consume all the bandwith available or the server will run very slow.
