Default Firmware - Hardware, Software.. or both?
A firmware is a piece of software that controls hardware (or something like that, right?) but can it be only a hardware thing? rather than having some sort of programming?

and is a BIOS a firmware?

BIOS is a firmware.

Whether you can get firmware outside of the CPU depends on your perspective. I've seen the C sections of some Lisp interpreters called microcode (which is a synonym for firmware), in that case it means that the microcode implements a virtual processor with an instruction set on top of the host processor. It's usage in such a case is questionable.
